Hello, my name is Ms. Jacquetta Evans, the School Clerical Specialist I at Azalea Gardens Middle School.  To request a parent/ teacher conference for your child, click here to schedule a meeting.  Conferences are held every Wednesday at 3:00 p.m. Please provide the parent or legal guardian and student’s full name, grade level, phone number, email address, and teacher/s name you are requesting a conference with.  I will contact you when the conference has been scheduled.  If you have any additional questions or concerns, please contact me via email at jevans2@nps.k12.va.us.
